Nomination Procedures - The Turner A. Blackburn Hall of Fame/Lifetime Achievement Award

  1. Any member of the AASPT may nominate candidates for the award.  A member may self-nominate. One (1) award will generally be given in any year. 
  2. All nominations should be submitted online through the Academy's website. Please do not submit materials via mail or email. 
  3. Nomination packets must include:
    1. Completed online nomination form; and
    2. Letter of support from the person nominating the member.
  4. The sub-committee will be selected by the current HOF members.  The committee shall select a maximum of three (3) awardees who meet the scoring criteria minimum and make these recommendations the Awards Committee.
  5. Please have all materials completed and collected when completing the award application.  There is no way to save applications in-process so please ensure you have all materials compiled before starting the application. 

The recipient shall give a brief acceptance for the award at the award ceremony. The recipient will be invited to write a written statement for potential publication by the AASPT.
