Nomination Procedures - Lifetime Excellence in Education Award 

  1. Any member of the American Academy of Sports Physical Therapy may nominate candidates for the award.
  2. A maximum of one (1) award will be given in any year.
  3. Nominations must be submitted electronically and include only the following materials:
    1. Completed online nomination form; 
    2. Current and complete curriculum vitae of the nominee; 
    3. Letter of support from person nominating the member not to exceed two pages including the reasons for the nomination; and,
    4. Letters from four (4) individuals who can specifically address one or more of the criteria for selection, not to exceed two (2) pages.  One should be from a physical therapist colleague and one from a current or former student/resident of the nominee.  The additional two (2) letters should come from individuals who can address the long term contributions of the nominee.  
  4. Please have all materials completed and collected when completing the award application.  There is no way to save applications in-process so please ensure you have all materials compiled before starting the application. 
The Awards Team Coordinator will notify the recipient of the award and obtain written confirmation of acceptance. The name of the recipient will be kept confidential until announced.
